Electric Public Utility Fee Report Forms


Pursuant to Indiana Code IC 8-1-6, on or before May 1 of each year, all public utilities as defined by IC 8-1-6-3 in the State of Indiana shall report to the Commission gross revenue including intrastate operating revenue. The report is to be sworn to by an appropriate officer of the public utility.

General Instructions for Submitting a Public Utility Fee Report

All forms must be submitted electronically using the Commission's Online Services Portal. Instructions on how to use the Online Services Portal can be found here.

  • Form must be completed in full and include a valid email address.
  • The form must include the Utility ID. To find your Utility ID, click here.
  • PLEASE NOTE: Forms are due no later than May 1.
