Electric Public Utility Fee Report Forms
Pursuant to Indiana Code IC 8-1-6, on or before May 1st of each year, all public utilities as defined by IC 8-1-6-3 in the State of Indiana shall report to the Commission gross revenue including intrastate operating revenue. The report is to be sworn to by an appropriate officer of the public utility.
General Instructions for Submitting a Public Utility Fee Report
- Form must be completed in full.
- Include the Utility ID#. Contact the Commission at Public Utility Fee Utility ID if you do not have your Utility ID#.
- Upon completion of the form, sign and return by:
