Asthma School Checklist

 

Give a copy of your child’s Asthma Action Plan to his or her teachers or care givers. Explain how to read the plan, medications and any medical devices your child needs to manage their asthma, and what to do during an asthma attack.
Supply the school with your child’s prescribed inhaler. Replace expired medications. Notify the school if medications are changed.
If your child can use their inhaler on their own bring in a written form signed by their doctor, Permission to Carry Asthma Medications to School Contract. This form must be filed with the school at the beginning of each school year.
Check the inhaler your child carries for medication often. Have a back-up inhaler. Replace expired medications. Notify the school if medications are changed..
□ Ask to tour the school to look for any asthma triggers. Talk to the principal about any problems you find.
□ Ask about the school’s pesticide policy. Will you receive notice when pesticides are applied? Does the school use an Integrated Pest Management program?
□ Does the school have an Indoor Air Quality (IAQ) policy? IAQ is the quality of air determined by the amount of airborne contaminants present. Indiana state law IC 16-41-37.5 addresses IAQ and investigations of complaints in schools.
□ Ask to see your child’s classroom and any other settings where your child might be throughout the day. Look for asthma triggers that could bother your child’s asthma.
□ Ask about the smoking policy at the school. Smoking SHOULD NOT BE ALLOWED where children will be exposed.
