Asthma Action Plan

A physician’s prescribed Asthma Action Plan for a child with asthma is essential. An Asthma Action Plan helps organize all the information about your child’s asthma. The Asthma Action Plan should be updated as your child’s needs or asthma severity change. The Asthma Action Plan should include your child’s asthma triggers and activities that may cause an asthma attack, signs and symptoms of an asthma attack, routine and emergency medication, medial devices used (such as a spacer), emergency contact information and procedures to follow in the event of an asthma episode. Copies of the Asthma Action Plan should be given to any person that will care for your child, such as the school nurse, your child’s teacher, school bus driver, athletic coaches, child care staff, babysitters, grandparents, etc.
