Reports on Health Care Facilities
The Indiana State Department of Health (ISDH) is responsible for the licensing of health care facilities. The Consumer Services and Health Care Regulation Commission within ISDH serves as the state survey agency for state licensing and federal Medicare/Medicaid certification. This web page is intended to provide access to reports prepared by the Commission pertaining to health care facilities.

Health Facilities Consumer Reports
In 2003 the ISDH created its first health care facility consumer report. That report was for nursing homes. The ISDH has since added additional consumer reports for other provider types. Each consumer report provides information about health care facilities and entities. The information includes location, contact information, administrators, services provided, and survey history.
The Indiana Medical Error Report provides the number of reported serious adverse events at hospitals, ambulatory surgery centers, abortion clinics, and birthing centers. The report is based on the National Quality Forum’s Serious Adverse Events. The report is produced annually.
The Indiana State Department of Health (ISDH) Consumer Services and Health Care Regulation Commission publishes two newsletters - the Infection Prevention Newsletter and the Long Term Care Newsletter. Both newsletters are electronic newsletters delivered via email and are available to anyone. The newsletters are free.
