The Office of Minority Health is now accepting volunteer applications for the 2018 Indiana Black & Minority Health Fair at Indiana Black Expo's Summer Celebration.

All volunteers will receive free admission and a health fair t-shirt. Please complete and submit the following form, making sure to fill-out all fields.

Please volunteer early due to limited time slots in each area.
