All students are required to have the following forms filled out prior to their first day of school. In an effort to streamline the process, documents are now provided online:
- Health Center Welcome!
- Medical Update Form
- Current Medication & OTC Authorization Form
- Dental Form
- Dental Form (Spanish)
- Special Dietary Needs
- Eye Report (will need to be done yearly)
- Adrenocorticoid Medication Instructions
Action Plans (only required if it applies to your child)
