Before requests for room reservations can be made, the requestor (individual) must have a Use Agreement on file with the Conference Center. The agreement outlines general rules of use, including restrictions as well as general Conference Center services. Once the Use Agreement is on file, it remains valid unless and until policy changes necessitate a revision.
Electronic versions of the Agreement are available below. After downloading the document, please review, complete, and sign. Then fax, mail or scan and email it to the Conference Center:

Conference@idoa.in.gov and request than an electronic version be sent to you. Alternatively, hard copies of the Use Agreement are available for pickup at the Conference Center.
