Classroom Pet Notice:
Asthma Management in Schools

I __________________________________, parent/legal guardian of _________________________ (child) am fully aware there is a _______________________ (type of pet) in my child’s classroom, and that animal dander can potentially be a problem for children with asthma and/or allergies. I have been informed the pet has all the necessary vaccinations, and animal cages are cleaned no less than three times a week. I have been assured that my child’s hands will be washed after handling, feeding, or cleaning pets or the animal’s enclosure.

Carpets are vacuumed daily with a HEPA-filter vacuum, non-carpeted floors are swept daily. Dense filtering material has been placed over air vents to trap airborne dander and animal cages are located away from air vents.

I have fully read the above information and discussed it with my child’s health care provider and feel this is not a concern for my child. I give permission for
________________________ (child) to play with and handle the classroom pet under proper supervision which follows state guidelines.
