On an annual basis, the Indiana State Department of Health requests home health agencies to identify the number of clients served during the calendar year, and some demographic characteristics of these patients. The information is collected by diskette, displayed on this site by county, and is updated annually. This report is for 239 home health agencies that filed in 2005.

As an alternative, you can click on a letter on the alphabet bar and see a list of the counties that begin with that letter. From that list, click on the county of interest. Each county profile will indicate the number of clients and the number of agencies serving clients in this county. Percentages of gender, age, treatment diagnosis, reimbursement, and source of admission and discharge are shown for counties with at least one Home Health Agency located in the county.
