Mobile Response Operations

Requesting an On-site Vaccination Clinic

DEP has 10 Mobile units that originated from the Covid-19 Mobile Response efforts. The mobile units are managed by the Local Emergency Operation section. Upon request, these units deploy to provide public health services including, but not limited to, routine immunizations, COVID-19 vaccination and testing, and HIV/STD testing.

  • How to Request a Clinic

    van exterior Anyone interested in hosting an on-site vaccination clinic should contact IDOH Immunizations staff at least 3 to 4 weeks ahead of the event.

    To request a Mobile unit for an event or site, submit a completed request form.

    • All vaccines, supplies, and staff needed to support the clinic.
      • Vaccines Available to Request:
        • Pfizer COVID Vaccine (Age 5 and up)
        • Pfizer COVID Bivalent Boosters (Age 5 and up)
        • Moderna COVID Vaccine (Ages 12 and up)
        • Moderna COVID Bivalent Boosters (Ages 12 and up)
        • Flu (Ages 3-64)
    • A registration link. We ask that you advertise the clinic and encourage pre-registration. Paper forms can be made available in both English and Spanish.
    What you will need to provide:
    • Instruction on where to park and which entrance to use
    • Access to 1 to 2 electrical outlets
