The Center’s mobile unit is available for assessment and consultation services throughout the state of Indiana. The unit includes an audiology suite with a sound booth, testing equipment, and a consultation area.

If you’re interested in a visit from our mobile unit, please start by reviewing these considerations:

• Collect completed Consent for Service forms. This form is available as a PDF (Spanish version) or electronic submission. This form allows you to share the intake information with us and is a needed step to have the child/student come onto the unit.
  • Sharing our Mobile Unit Flyer with families along with the consent form assists in understanding the purpose of our visit.
• Once you have completed consent for service forms, please fill out the Mobile Unit Request form for processing a requested visit.

We may be able to fulfill a request for onsite audiology needs without bringing our mobile unit, saving time and cost. Our audiology team will work with you on planning and implementation of these services through the above intake process
