1 Sep 3, 2013 4:35 PM Area Agency on Aging system: Care transitions initiatives; opportunity to implement the Community Living Program model statewide; demonstrated ability to work one-on-one and face-to-face with a fragile consumer population; deep and long-lasting community partnerships that we draw on to meet consumer needs; extensive existing information and referral systems; established provider networks and experience in their management; ability to empower consumers in their own care and to maximize resources for care;  as mission-driven non-profit agencies, the incentive to be fiscally responsible with all funds so that we can serve as many consumers as possible;
