All visitors to Logansport State Hospital must enter the hospital through the Communication Center lobby and complete a visitation form/pass. Visitors must also provide information for COVID contact tracing.
Visitors are required to use hand sanitizer and wear the personal protection mask provided by LSH at all times during the visit. Patients will also be required to wash their hands and wear the provided personal protection mask during the visit.
Visitors and patients will be required to maintain social distancing of six feet or more during the entire visit. There will be designated areas for visitors marked off to allow for social distancing. Visitors and patients are to sit across from each other. A violation of this rule will result in the termination of the visit and/or a 30-day visitation restriction.
Visits are not allowed during the first seven days following admission unless special circumstances are present and visitation arrangements have been made by the patient’s treatment team.
Each patient may have up to one visit within a 7-day period (Monday through Sunday).
Visits must be scheduled at least 24 hours in advance.
Visiting hours are from 2 p.m. to 4 p.m. daily.
A visit may be scheduled no more than one week in advance. An exception could be a visit scheduled after a Treatment Team meeting.
The Communication Center will schedule visits for all patients on Isaac Ray Treatment Center.
The Larson unit secretary will schedule visits for all patients on Larson Treatment Center.
No more than three patient visits will be allowed to be scheduled at one time at Isaac Ray Treatment Center.
No more than two patient visits will be allowed to be scheduled at one time at Larson Treatment Center.
Special “no contact” visits may be recommended by the treatment team based on patient level of severe behavior or violations of contraband rules.
All visitors must be 18 years of age or older unless prior approval has been granted by the treatment team. LSH reserves the right to exclude a visitor for whom proper and acceptable proof of age cannot be produced.
No more than two visitors will be allowed per patient at a time.
All visitors must secure personal belongings in an assigned locker.
Visitors may not bring gifts to the patients during their visit. All gifts must be shipped to the Logansport State Hospital for sanitization by the LSH Materiel Management Department.
No vending, food or drinks is allowed during the visit.
All patient visitors will pass through the metal detector prior to being escorted to visitation location.
Patients are to use the restrooms on the units before starting a visit. If there is a need to use the restroom during a visit, the patient will be returned to the unit and the visit will terminate. Special circumstances may exist for specific patients that may result in an exception to this rule.
A detailed search will be completed of all patients upon return to the unit.
All visits are supervised by LSH staff. Staff will position themselves to have visual contact with all visitors and patients.
Visits are limited to one hour.
Vehicles must be locked at all times while unoccupied on hospital grounds.
To protect the confidentiality of our patients, no cell, cameras or electronic/recording devices are allowed on hospital grounds.
Weapons are not allowed on hospital grounds. If you have a valid permit and carry a weapon, the item must be removed and locked in the trunk of your vehicle before entering a patient building.
ADA service animals only are allowed on hospital grounds.
Logansport State Hospital grounds are smoke and tobacco free.
