New Hire Processing
Forms are due to DWD HR no later than end of day on the first Wednesday of hire. All forms must be completed in black ink. Unless noted otherwise, all items must be completed by all new hires and rehires. Current intermittent employees promoting to regular employees will complete only benefits items.
- New Employee Onboarding
- Credential and Access Request Form (Badge Request)
- Pay Period Accrual Month
- Indiana Authorization for Direct Deposit | State Form 43591 (Current State Employees transferring to DWD from another agency need not complete)
- Provide HR with the original only.
- State Benefits Information Page: (Not applicable to Hourly/Intermittent Employees and current State Employees transferring to DWD from another agency)
