Employer Qualification and Special Circumstances

Pre-Registration Form

Before you register, please complete this questionnaire to determine whether you qualify and are required to register.

Registration

If you meet any of these criteria, you must register with DWD, usually through the Uplink Employer Self Service (ESS) portal.  You will then be assigned a State Unemployment Tax Account (SUTA) number.

Additional Notes

  • You must file quarterly wage reports, even if you have no wages reported during that quarter.
  • If you cease business operations or no longer have employees, you may apply to terminate your account . To terminate your employer account, please complete State Form 46800.
  • It is important that you continue to file quarterly wage reports until DWD notifies you that your account has been terminated or inactivated.
