Accessing the Employer Self Service (ESS) Website

Changing Addresses and Phone Numbers

Changing the legal mailing address or name is reported through the UPLINK / E S S web application or by filing SF2837 (SUTA Account Number Registration and Disclosure Statement) which can be downloaded from https://www.in.gov/dwd/2406.htm.

Sign In to ESS.

Once on the Dashboard, Select Profile Maintenance from the Left-hand Drop-down Bar.

Select Legal Information to Change the Mailing Address or Phone Number

Select Locations to Add/Change/Remove Worksite and Benefits Information.

Scroll Down to Primary Mailing Address.

Make Changes to Address or Phone Number.

Check the Box indicating the above address is the correct legal address.

Select Update.

Select Add another Location or Edit.

Select Update.
