Identity Confirmation

Overview

Identity theft occurs when someone obtains sensitive personal information—such as passwords, Social Security numbers, or credit card or bank account numbers—to defraud or commit crimes.

Victims of identity theft may lose significant money and time and may find their reputation and credit rating has been damaged, affecting the ability to obtain loans for education or housing, approval for rental agreements, and approval for credit cards or large purchases requiring credit. The criminal could also submit a fraudulent tax return and use children’s stolen identities in their scheme.

ID Passcode Confirmation

If you received a letter from DOR to complete an ID Passcode, you will need to make sure the name(s) and address information at the top of the letter matches an Indiana income tax return you recently filed. If the information is not accurate, you may call DOR at 317-232-3425, Option 1, Monday through Friday, 8 a.m.–4:30 p.m. ET for assistance.

Otherwise, you will only need to complete a simple Identity Confirmation Process which will help to protect your refund and identity. Once the identity confirmation process is complete, DOR will be able to continue processing your return. You should allow up to three weeks to receive your refund.

ID Confirmation Process

This first person is considered the primary taxpayer and the second person is considered the spouse.

To complete the Identity Confirmation Process, you’ll need the following:

  • Your Indiana tax return for the tax year designated on the letter
  • Your ID Passcode letter

You only have three attempts to complete this step prior to the deadline provided in your letter. Your return will not process until you have completed this step.

Protect Yourself from ID Theft

Here are some simple, but important tips to protect your information:

  • Protect your financial documents that include your name, address, bank information, and Social Security number. Keep important papers locked in a secure place and shred unneeded documents.
  • Don’t provide your Social Security number just because they ask.
  • Don’t carry your Social Security card with you.
  • Check your credit report every 12 months for free by directly contacting the three credit reporting agencies: Experian, Equifax and TransUnion.
  • Protect your personal computers by using firewalls, anti-spam/virus software, update security patches, and change passwords periodically for internet accounts.
  • Don’t provide personal information over the phone, by mail, or online—unless you are sure you know the requestor.

Report ID Theft

If you are a victim of identity theft, report it to DOR and provide the following information:

  • Completed IRS Identity Theft Affidavit (Form 14039)
  • Letter from the IRS stating you may be the victim of ID Theft (if applicable)
