The Indiana Department of Revenue (DOR) continues to implement increased security measures to protect Hoosiers and the state from identity theft and identity fraud. Since its inception in 2014, the Identity Protection Program has stopped more than $110 million in fraudulent refund attempts.
How Identity Confirmation Works
If you received a letter from DOR, you first will need to make sure the name(s) and address information at the top of the letter matches an Indiana income tax return you recently filed. If the information is not accurate or you have not filed an Indiana tax return, you may call DOR at 317-233−1642, Monday through Friday, 8 a.m. - 4:30 p.m. EST for assistance.
If the information at the top of the letter does match your recently filed Indiana income tax return, all you need to do is complete a simple Identity Confirmation Quiz. This quiz should take no more than two minutes and will help to protect your refund and identity. Once your quiz is complete, we will be able to continue processing your return.
Get Ready for the Quiz
The quiz is based on information associated with either name listed on the letter you received. This first person is considered the primary taxpayer and the second person is considered the spouse. You will be given the option to choose which person is attempting the quiz.
What you'll need to complete the Identity Confirmation Quiz:
- Refund amount on your Indiana tax return
- Tax year indicated on your Identity Confirmation Quiz Letter (underlined in the second paragraph)
- Document Locator Number (DOC LOC) located on your Identity Confirmation Quiz Letter
Remember you only have three minutes to complete the quiz. You get two attempts to complete the quiz online before you are required to contact DOR.
