Roll of Attorneys Forms

Designation of attorney surrogates, changes to attorney contact information, and submission of affidavits of inactive or retired status are now accomplished online through the Clerk of Courts Portal.

The following forms are available in Microsoft Word and Adobe PDF format only and are made available by the Clerk of the Indiana Supreme Court, Court of Appeals, and Tax Court. These PDF and Word documents are fillable forms, which means you can type your responses directly into the form before printing it out.
