Alcohol Permit Applications & Forms

The Indiana Alcohol and Tobacco Commission (ATC) is happy to provide guidance and direction on the process for submitting applications and obtaining alcohol permits or tobacco, e-liquid, and type II gaming certificates. However, the ATC cannot provide legal or business advice. Please consult a private attorney if you have specific legal questions related to an application form.

Additionally, the ATC cannot accept an application form until it is fully completed and all required documentation is submitted, including payment. If an application form is incomplete or missing supporting documentation, the application along with payment will be returned to you. Once a fully completed application is submitted, the application will be scheduled for a local board hearing, and a Commission meeting. The ATC recommends applying for a permit at least ninety (90) days in advance of any deadline. The ATC cannot work around opening dates or events scheduled before a permit is issued.
