Requested dates for use of White River State Park cannot be confirmed until this signed application, certificate of insurance, and all fees are received. All applications are subject to review by the White River State Park Development Commission staff. If the requirements of this application are met and a date confirmed, acknowledgment will be transmitted within seven business days.
