The state of Indiana’s contract with Welborn HMO will expire December 31, 2010. Your coverage will remain in place through the end of 2010, but you will need to make a new selection for 2011 during open enrollment this fall. Failure to sign up for a new health plan during open enrollment will result in no coverage in 2011.
With the loss of this plan it is important that you review the other three plans to find the one that will work best for you and your family. The state currently offers three statewide plans: CDHP 1, CDHP 2 and Traditional PPO. Each plan has differences in premium costs, deductibles and out-of-pocket maximums. However, all three offer 100 percent preventive coverage. That includes physicals, well baby checks, mammograms, prostate exams, etc.

Welborn HMO Plan is available to participants that live or work in the following counties: Daviess, Dubois, Gibson, Knox, Perry, Pike, Posey, Spencer, Vanderburgh, and Warrick. Like the other plans, Welborn subscribers can reduce their bi-weekly premium by $10 by selecting the $10 non-tobacco use incentive.
