Instructions to Print a Dental ID Card
Delta Dental does not send ID cards so you will need to print one by following the instructions below. Your provider will need to see your ID card when you receive services.
Go to www.deltadentalin.com/ and click on the Consumer Toolkit link. In the next screen, you will need to complete your Social Security Number, and date of birth. On the left side of the next screen, click on the Print ID Card link. Individual ID cards are not available for dependents.
After you have printed your card, be sure to log out.
