Name and/or Address Change

Name Change

  • Completed application
  • Certificate of Insurance or SR-23 with new name shown (can only accept Certificate of Insurance for boat dealer name change).
  • Pictures of new sign with new name.
  • Copy of Retail Merchants Certificate with new name.
  • Fee is $5.00 for new license and $2.00 for each plate registration.

Address Change

  • Completed application
  • Certificate of Insurance or SR-23 with new address shown (can only accept Certificate of Insurance for boat dealer name change).
  • Pictures of building, lot and sign.
  • Copy of Retail Merchants Certificate with new address.
  • Bottom of application completed by local zoning board.
  • Fee is $5.00 for new license and $2.00 for each plate registration.

911 Address Change

  • Certificate of Insurance or SR-23 with new name shown (can only accept Certificate of Insurance for boat dealer name change).
  • Statement from Planning Commission showing 911 change.
