- Next Expiration Date: January 1, 2013
- Course Provider Expiration Date: December 3, 2012
If your license has expired and you wish to renew please email pla11@pla.in.gov with your name and license number to request a renewal form. You will need to complete and return the form, proof of insurance, and the renewal fee and late fee.
*Online license renewal is not available for this renewal period. Renewal forms were sent through U.S. Mail on November 1, 2012. If you have not received a renewal form please contact our office as soon as possible.
