Error Correction Sheet Use this document as a guide to correcting errors in your uploads. Records with errors do not make it into the database, they must be corrected and resubmitted to remain compliant with the Indiana Board of Pharmacy.
Online ZERO REPORT Form Please use the online form to complete your zero reporting to the INSPECT program.
Pharmacy Information Change Order Form
Fill out this form if your pharmacy location has incorrectly reported information to the INSPECT database. This can be used to correct records if your pharmacy has already uploaded information. The INSPECT program will only make changes after the Change Order Form has been submitted, no exceptions.
