OBTAINING A LICENSE TO POST AT YOUR FACILITY:
-
We no longer print and mail licenses for funeral director interns, funeral directors, funeral homes, funeral home branches, cemeteries, or certificates of authority free of charge. This does not change the license posting requirement where applicable. The licensee will be required to go to our website at www.pla.in.gov to utilize our License Express feature to order/print a license. You may print a free license from your computer or order the blue cardstock license for a fee of $13.77.
LICENSE CERTIFICATION/VERIFICATION: The State of Indiana has changed the method it uses to verify licensure for Indiana licensees. The new procedure will allow anyone to request a digitally certified certification letter and license status information that can be forward to any licensing board or individual. What they will receive is a pdf file with two pages - the letter and the status page. Both will have a US Postal mark to verify authenticity as of the date it shows. This process is admissible in all courts. There will also be a web link in the letter that allows you to verify that the file is authentic, digitally signed, and has not been modified. Paper certifications will no longer be provided. To use the digital certification function, please click on "License Express" at the top left corner of this page.
COURTESY CARDS FOR BORDERING STATE FUNERAL DIRECTORS: Effective January 1, 2013, Senate Enrolled Act No. 370 (IC 25-15-10) provides for a courtesy card for bordering state funeral directors to perform limited practice in Indiana providing their state also offers the same to Indiana funeral directors. Click HERE to view IC 25-15-10. Kentucky is currently the only state that also issues a courtesy card to Indiana funeral directors. Click HERE for the Courtesy Card Application.
