Renewal Information

Massage Therapist Certifications expire May 15 of every fourth year (2017, 2021, 2025)

  • The massage therapist renewal fee is $150.
  • Certified massage therapists are required to hold professional liability insurance in order to practice massage therapy in the state of Indiana.

Renewal notices are sent approximately ninety (90) days prior to the expiration date. Certificate holders with valid email addresses on file will be emailed the renewal notice. Those who do not have valid email addresses on file will be mailed the license renewal notice; this notice is mailed to the address of record with the Board. The Board has no way of knowing whether or not a notice reaches its destination; therefore when a notice has been emailed to a valid email address or mailed, the duty of the Board has been performed.

Online renewal is available 24 hours a day, 7 days a week for most license renewals.

Paper Renewal Documents

To renew your license by paper, you must select a document below. Print and mail the completed form with the required renewal fee and proof of insurance to the Indiana Professional Licensing Agency. The address is on the form, and checks should be made out to 'Indiana Professional Licensing Agency.' If the  renewal form you need is not available, please contact the Board by email at pla14@pla.in.gov.

Required Documentation

You are required to hold professional liability insurance in order to practice massage therapy in the State of Indiana. The proof may be a notarized copy of professional liability insurance or proof submitted directly from the insurance provider. In order to finish your renewal, you must send proof of insurance to the board with your paper renewal application or by email to pla14@pla.in.gov. If you submit your renewal application online you will still be required to submit this documentation before the renewal is processed. Please include your certification number on all documentation. Once your documentation has been evaluated and your renewal completed you will receive a confirmation email with instructions on printing a new certificate.
