HFA and RCA licenses expire on August 31 of even-numbered years. The next renewal date is AUGUST 31, 2014. Please see the information on continuing education.
ONLINE RENEWAL - A license renewal notice is emailed to the licensee's email address of record with the Professional Licensing Agency sixty (60) days prior to the expiration of the license. Information is provided in the notice on how to renew your license online. If you do not receive your renewal notice, please contact our office by calling (317) 234-2051 or by email at email@example.com. Please be sure to give us your name, current address, license number, profession, email address and telephone number.
Address Change - You are required to notify the Professional Licensing Agency of any address changes. You can update your address during the renewal process or anytime using our License Express option. You may also notify us in writing. Your written notification must include your name, license number, profession, social security number, date of birth, and both your old and new addresses. Please send the information to the Professional Licensing Agency at 402 West Washington Street, Room W072, Indianapolis, Indiana 46204, e-mail at pla6@pla.IN.gov, or fax it to (317) 233-4236.
Name Change - You may have the name changed on your licensure records by submitting a copy of an official name change document such as a marriage certificate, divorce decree, or other legal name change document. All documents must be notarized. Please send the information to the Professional Licensing Agency at 402 West Washington Street, Room W072, Indianapolis, Indiana 46204. You also need to include your name as it is listed now, your new name, your license number, profession, social security number, and date of birth. If you would like an updated pocketcard, you can request one online using our License Express option.
NEW RULES REGARDING INACTIVE STATUS!!! Effective July 5, 2008, you can renew your license on an inactive status. Please note that you cannot practice while on inactive status. Please refer to the new rule for information on how to reactive your license once you place it on inactive status.
