Renewal Information / Address Change / Name Change

Renewals
Veterinary Licenses and Controlled Substance Registrations (CSR) expire on October 15 of odd numbered years. The next renewal date is OCTOBER 15, 2009.

Veterinary Technician Registrations expire on January 1 of even numbered years. The next renewal date is JANUARY 1, 2010.

Continuing Education
Continuing education is also a requirement for renewal for veterinarians and veterinary technicians. Please see specific information on the Board's website regarding http://www.in.gov/pla/2650.htm.

Licensure Renewal Information
Licensure and registration renewal information is mailed to the licensee's or registrant's address of record with the Indiana Professional Licensing Agency sixty (60) days prior to the expiration of your license or registration. Information is provided on how to renew your license/registration online.

Online Renewal
You can renew your license/registration online at http://www.in.gov/pla/license.htm.

  • QUICK & EASY. It takes just minutes and your renewal will be processed and available for verification the next business day. 
  • Available 24 hours a day, 7 day a week.
  • Go to License Express.  Just use your license number (ex: 24009999A) as your Login ID and your social security number as your Password.  Do not put dashes or spaces in for your social security number (ex: 999999999).
  • You can update your address, telephone number and email address as part of the renewal process.
  • Pay your renewal fee(s) by using your Visa, MasterCard or Discover credit or debit card.  There are access and credit card fees associated with renewing on-line.  No part of these fees come to IPLA.   

Renew By Mail
If you prefer to renew by mail, please contact the Veterinary group at (317) 234-2054 or email us at pla8@pla.IN.gov to receive a printed renewal form.  Please be sure to give us your name, address, license/registration number, profession, and telephone number.

Failure To Renew
Failure to renew a license or registration on or before the expiration date automatically renders the license or registration invalid without any action by the Board.

Renewal Fees
Veterinary License - $100.00

Controlled Substance Registration (CSR) - $60.00

Veterinary Technician Registration - $15.00

Late Renewals - Penalty Fees
A Veterinarian or Veterinary Technician may renew a license and/or registration by paying a late fee of $50.00 in addition to the renewal fee.

Veterinary License - $150.00.

Controlled Substance Registration (CSR) - $110.00

Veterinary Technician Registration - $65.00

You can determine if your renewal has been received and processed by calling our toll free line at 1-888-333-7515 or by accessing our free online verification search.

Address Change
You are required to notify the Indiana Professional Licensing Agency of any address change. Please call our office at (317) 234-2054 or by email at pla8@pla.IN.gov or by FAX at (317) 233-4236 to report a change of address. You can also submit your address change in writing to the:

Please be sure to give us your name, license/registration number, profession, social security number, date of birth, and both your old and new addresses.

Name Change
You may have the name changed on your licensure or registration records by submitting a legal name change document (marriage certificate, divorce decree, social security card or other legal name change documents).

Your request to change your name should include your name as it is listed now, your new name, your license or registration number, profession, social security number, and date of birth. If you wish to have a new pocketcard, please indicate in the letter that a new pocketcard is needed due to the name change.
