LATE RENEWALS -- PENALTY FEES
If a renewal application is postmarked after March 31, 2014 the license automatically becomes invalid. The license may be renewed within three (3) years of the date of expiration by paying a late fee of $50.00 in addition to the normal renewal fee of $50.00 and completion of the continuing education requirement.
If you have been expired for longer than three (3) years, you will need to contact the Board directly to request a reinstatement application form. You will be required to pay the renewal fee and late fee, submit a detailed letter of explanation to the board as to why your license has lapsed, submit proof of having met the continuing education requirements for one (1) renewal cycle within the previous twenty four (24) months. You may also be required to make a personal appearance before the board or pass the examination currently required to obtain initial licensure pursuant to 839 IAC 1-2-2(f)(7). ***Please note that if you received your initial licensure through the grandfathering process, the board will require you to take and pass the licensure examination and jurisprudence examination prior to renewing your license***
The renewal application must be postmarked by March 31, 2014 to avoid the payment of any late fee.
For renewal applications postmarked after March 31, 2014, a late fee will be assessed for late renewal.
Upon completion of the renewal process, your license to practice will be current through March 31, 2016.
