Question:  How do I request a duplicate pocketcard and is there a fee?

Answer:  You may submit your request to pla4@pla.IN.gov or faxing (317) 233-4236.  This request must also include your name, license number and current mailing address.  There currently is no fee for a duplicate pocketcard.

Question:  How do I request a duplicate wall license (applicable to pharmacists only) and is there a fee?

Answer:  You may submit your request in writing to Professional Licensing Agency, 402 W Washington Street, Room W072, Indianapolis, Indiana, 46204.  This request must also include your name, license number and current mailing address.  This may only be sent to our office by mail as there is a $10 processing fee.  This fee needs to be in the form of personal check or money order made payable to "Professional Licensing Agency".

Question:  I need my license/permit/certification/registration verified to another state board or association, how do I request this and is there a fee?

Answer:  You may submit your request in writing to Professional Licensing Agency, 402 W Washington Street, Room W072, Indianapolis, Indiana, 46204.  This request must also include your name, license number, and where the verification needs to be mailed.  This may only be sent to our office by mail as there is a $10 processing fee.  This fee needs to be in the form of personal check or money order made payable to "Professional Licensing Agency".

Question:  How do I update my address?

Answer:  If you hold a pharmacist license, intern registration, technician or technician-in-training permit, you may submit your request to pla4@pla.IN.gov or faxing (317) 233-4236.  This request must also include your name, license number and old and new mailing addresses.  If you are a facility, you must submit a new application to update your address.  Please see the licensing requirements for your particular profession for further information.

Pharmacists:

Question: As a "floater" pharmacist, do I need to publicly display my license in accordance with the Blue Card Rule at each store where I work?

Answer: Yes. There are three acceptable alternatives for "floater" pharmacists. You may do one or more of the following to meet the Blue Card Rule (See Blue Card Rule -- New!)You may (1) request duplicates and display the license in each store where you work; (2) post the license at the beginning of your shift and then remove it when the work day is over; or (3) wear the license on your lapel in lieu of your nametag without displaying it with the other pharmacy licenses.

Question: Am I required to offer counseling to every patient for each prescription I fill?

Answer: Yes. There are no exceptions to the rule irrespective of patient familiarity, the safety of the medication, or refilling. As stated in 856 IAC 1-33-2 (a), "Upon the receipt of a prescription or upon the subsequent refilling of a prescription, and following a review of the patient's prescription medication profile, the pharmacist shall be responsible for the initiation of an offer, as set forth in section 1.5(a) of this rule, to counsel the patient on matters that, in the pharmacist's professional judgment, are significant to optimizing drug therapy."

Question: As the qualifying pharmacist, am I required to notify the board when I am no longer employed by the pharmacy as its qualifying pharmacist?

Answer: Yes. When you are no longer employed by the pharmacy as the qualifying pharmacist, you must complete the "Change of Qualifying Pharmacist" available on the board web site at http://www.in.gov/icpr/webfile/formsdiv/01572.pdf and submit the form to the board. Please refer to 856 IAC 1-15-1, "If a qualified pharmacist, who, having upon the basis of his or her qualifications caused a pharmacy permit to be granted to any person, firm, corporation or co-partnership desiring to operate, maintain, open or establish a pharmacy should leave the employ of such pharmacy, he or she shall immediately notify the Indiana board of pharmacy (board) and the owner shall file an application with the board to qualify the permit with another pharmacist."

Question: As a recent pharmacy school graduate, what steps do I take to receive my pharmacist license?

Answer: Please complete and submit the "Application for Pharmacist's License" which can be downloaded at http://www.state.in.us/icpr/webfile/formsdiv/36028.pdf, along with the $100 application fee, tothe Indiana Board of Pharmacy. You may thenregister forthe NAPLEX andMPJE at http://www.nabp.net/.Once we receivethe completed application and fee and the"Certificate of Completion" form from your school of pharmacy,wecan approve yourapplication and approve you to take the exams with the NABP. Once the NABPreceives the approval from the Board, the NABPwill send your "Authorizationto Test" (ATT) andinformation on how to schedule your exams.

Question: May a prescription security blank have more than one (1) prescription written per blank?

Answer: No. Indiana law strictly prohibits multiple prescriptions on one (1) security blank. Please see 856 IAC 1-34-2 (7), which states, "Only one (1) prescription may be written per prescription blank. The following statement must be printed on the bottom of the pad: 'Prescription is void if more than one (1) prescription is written per blank.'."
