All permits and licenses issued by the Board are renewed every two years unless otherwise noted:
Renewal applications are mailed 60 days prior to the expiration of the license or permit. If you do not receive your renewal application at that time, or have any questions, please contact our office at (317) 234-2067 or by e-mail at pla4@pla.IN.gov. Please be sure to give us your name, current address, and license number. You may also renew your license by coming into our office at 402 West Washington Street, Room W072, Indianapolis, Indiana 46204.
New Renewal Requirements for Pharmacist Interns
Your paper renewal application must be accompanied by renewal fee along with one of the following: (1) Notarized copy of your Foreign Pharmacy Graduate Examination Committee (FPGEC) Certificate, OR (2) Official transcripts from an American Council of Pharmaceutical Education (ACPE) accredited school of pharmacy verifying your active enrollment in the professional program or graduation. This requirement was established in IC 25-26-13-10(c). Because of the documentation that is required to be submitted with the renewal form, there is no option to renew online.
New Renewal Requirements for Wholesale Drug Distributors
Your paper renewal application must be accompanied by renewal fee along with your Verified Accredited Wholesale Distributors (VAWD) accreditation number. If you do not currently have a VAWD number, but are in the process of becoming VAWD accredited, verification of application may be submitted in lieu of having the VAWD number. For further information regarding VAWD accreditation and WDD licensure, please visit our website at http://www.in.gov/pla/2562.htm. Because of the information that is required to be submitted with the renewal formthee is no option to renew online.
Address Change
You are required to notify the Professional Licensing Agency of any address changes.
If you are a practitioner, you may submit your change of address on your paper renewal application, or, if applicable, your online renewal appication. You may also submit your change of address at any time online using the License Express system or by submitting a written notice to the Board. Your written notice must include your name, license number, and both your old and new addresses. Please send the information to the Professional Licensing Agency at 402 West Washington Street, Room W072, Indianapolis, Indiana 46204, e-mail at pla4@pla.IN.gov, or fax it to (317) 233-4236.
If you are a facility, you must submit a new licensing application, required documentation, and, for in-state facilities (except WDDs), submit to an inspection by a compliance officer. The renewal application and renewal fee must also be submitted in order to renew the license.
Name Change
You may have the name changed on your licensure records by submitting a copy of an official name change document.
If you are a practitioner, you must submit a marriage certificate, divorce decree, or other legal documentation in order to make the name change. Please send the information to the Professional Licensing Agency at 402 West Washington Street, Room W072, Indianapolis, Indiana 46204, e-mail a copy to pla4@pla.IN.gov, or fax the information to (317) 233-4236. Your request should include your name as it is listed now, your new name, and your license number. If you want us to send you a new pocket card, please indicate that a new card is needed due to the name change.
If you are a facility, you must submit legal documentation verifying the name change. Please send the information to the Professional Licensing Agency at 402 West Washington Street, Room W072, Indianapolis, Indiana 46204, e-mail a copy to pla4@pla.IN.gov, or fax the information to (317) 233-4236. If the name change is due to a change in ownership, you must submit a new licensing application and required documentation. The renewal application and renewal fee must also be submitted in order to renew the license.
