New fees for Physician Applicants
Effective Date: March 13, 2002
Cost of Application for licensure:
$250.00 application fee payable to the Professional Licensing Agency
Renewal of License:
$200.00 every two years, due by June 30 of odd-numbered years
Temporary Medical Permit:
$100.00
Renewal of Temporary Medical Permit:
$50.00
Renewal of license that has been expired for up to three years:
$200.00 + $50 late fee
Renewal of license that has been expired for more than three years:
