VERIFICATION LINE & FREE ONLINE VERIFICATION
The Indiana Professional Licensing Agency will verify the status of a license by calling (317) 234-2054 or you may obtain basic information on a practitioner's license for free by accessing our free online verification search.

VERIFICATION OF LICENSURE TO ANOTHER STATE
This is a verification of an Indiana license to another state.   The state of Indiana is now providing a digital certification of the license that you can download and email to your state board.  This is an official statement of the basis of licensure and includes the practitioner's name, profession, license number, status and any other conditions/restrictions of the license.  Click the link below to order your digital certification.

INITIAL AND ADDITIONAL LICENSE CARDS
Initial and additional license cards may be obtain at Services.IN.gov/License Express.   You may purchase a blue license card to be mailed to you or download a free license card for immediate printing.  

LARGE WALL CERTIFICATES
If you wish to purchase a large wall certificate you have the opportunity to do so by contacting our vendor at the information listed at http://www.in.gov/pla/files/IPLA-Certficates.pdf.

CONSUMER COMPLAINTS
Complaints against a practitioner are investigated by the Office of the Attorney General - Consumer Protection Division.  Information concerning the filing of complaints may be obtained by calling 1-800-382-5516 or (317) 232-6330 or at the Attorney General Website.
