Application fee:

Renewal of licensure fee:
$50.00 every two years, due by December 31st of even-numbered years
Renewal of licensure that has been expired for up to three years:
$50.00 late fee plus the current renewal fee.
Renewal of licensure that has been expired for more than three years:
Please contact the board office for information.
Temporary permit fee:
$25.00
Verification of licensure status to another state:
$10.00 for each state to which information is to be sent
