William H. Maughn enlisted in Company G, 11th Indiana Volunteers Regiment on April 19, 1861, at Ladoga, Indiana. He served for three months, and was mustered out on August 4, 1861. On August 31, 1861, the 11th Indiana was reorganized and mustered in for three years’ service, with Lewis Wallace as colonel.

One certificate with letterhead “Executive Department Indiana Adjutant General’s Office” form with blanks filled in and the date stamped March 18, 1907.
“This certifies that the official records of which I am the lawful custodian on file in this office show that William H. Maughn joined for duty and was enrolled as a Private of Company “G,” 11th Regiment Indiana Volunteers at Ladoga on the 19th day of April 1861, by Captain H. M. Carr and that he was duly mustered into the Military Service of the United States at Indianapolis on the 22nd day of April 1861 for the term of three months by Major T. J. Wood, Mustering Officer. Age given as 20 years. Mustered out Aug. 4, 1861 term expired. This Certificate is given as official evidence of enlistment service and muster out of William H. Maughn of Company “G” 11th Regiment, Indiana Volunteers. Witness my hand and official seal. Oran Perry, Adjutant General, Indiana.”
