Terms and Conditions
I, the undersigned, hereby represent that the artwork described above is an original work of authorship personally created by me to which I am entitled copyright protection. In consideration for the acceptance of the artwork by the Office of the Indiana Lieutenant Governor, and intending to be legally bound hereby, I grant the Office of the Indiana Lieutenant Governor and the State of Indiana the right to publicly display the artwork, if it is selected for display, for a period of two years from the latest date on this form. I acknowledge that the final decision regarding the suitability of the artwork to be displayed will be made by the Office of the Indiana Lieutenant Governor. I also grant the Office of the Indiana Lieutenant Governor and the State of Indiana the right to reproduce the artwork for any non-commercial purpose. I further grant to the Office of the Indiana Lieutenant Governor and the State of Indiana the right to include or authorize the inclusion of the artwork in a compilation or derivative work in any form, including, but not limited to, book, pamphlet, website, or electronic format, for the benefit of the Office of the Indiana Lieutenant Governor or the State of Indiana. I hereby release the Office of the Indiana Lieutenant Governor, the State of Indiana, their employees, agents, and assigns from any and all liability for damage, loss, or misappropriation of the artwork. Notwithstanding the grants of rights above, I retain all rights, including, but not limited to, legal title and copyright, in the artwork. I further agree to indemnify, hold harmless and defend the Office of the Indiana Lieutenant Governor, the State of Indiana, their employees, agents and assigns against any and all claims of any nature whatsoever, including, but not limited to, claims of copyright infringement, by any party whatsoever, arising out of or in any way related to the submission of the artwork. I acknowledge that if I do not retrieve the artwork at the end of a period of two years from the latest date on this form, the artwork will become sole property of the State of Indiana and any and all rights retained by me will terminate.

Electronic image of artwork must be submitted via e-mail or CD to Karen Davis. (CD will not be returned unless accompanied by a self-addressed stamped envelope.) The artist’s name and contact information must be included in the email of the art submission.
