TITLE 410 INDIANA STATE DEPARTMENT OF HEALTH

Notice of Intent to Adopt a Rule

Under IC 4-22-2-23, the Indiana State Department of Health intends to adopt a rule concerning the following:

OVERVIEW: Adds 410 IAC 18-0.5 to add definitions. Amends 410 IAC 18-1 to correct terminology. Adds 410 IAC 18-3 to specify the documentation required to obtain a certified copy of a birth record and to whom a birth certificate may be issued by the state registrar. Adds 410 IAC 18-4 to specify the documentation required to obtain a certified copy of a death record and to whom a death certificate may be issued by the state registrar. Written comments may be submitted to the Indiana State Department of Health, Operational Services Commission, 2 North Meridian Street, Indianapolis, Indiana 46204. Statutory authority: IC 16-19-3-4; IC 16-19-3-5; IC 16-37-1-3.
