IC 4-22-2-38, corrects the following inaccurate references in the Indiana Administrative Code:
(1) In
405 IAC 5-1-5(c), after "On", delete "UB-92" and insert "UB-04".
(2) In
405 IAC 5-1-5(c), after "use the appropriate", delete "UB-92" and insert "UB-04".
