Under IC 4-22-2-24, notice is hereby given that on February 10, 2023, at 10:00 a.m., at the Indiana Government Center South, 402 West Washington Street, Conference Center Room 3, Indianapolis, Indiana, the Office of the Secretary of the Indiana Family and Social Services Administration – Division of Family Resources will hold a public hearing on a proposed rule adding 470 IAC 15 regarding submission of claims to defray expenses for funeral and cemetery providers.

Pursuant to IC 4-22-2-24(d)(3), the Indiana Family and Social Services Administration (FSSA) – Division of Family Resources (DFR) proposes to add 470 IAC 15 to provide that funeral and cemetery providers must submit the claim to defray funeral and cemetery expenses form, state form 35937, to the DFR within 90 days of the date of death of the decedent to obtain reimbursement for funeral and cemetery expenses.

All parties interested in the proposed rule are invited to attend the public hearing and to offer public comments. In lieu of attendance at the public hearing, written comments may be sent to: FSSA, Attention: Amanda DeRoss c/o Katie Hunter, 402 West Washington Street, Room W451, Indianapolis, IN 46204. Correspondence should be identified in the following manner: "COMMENT RE: LSA Document #22-142". All written comments concerning the rule received by the FSSA – DFR will be available for public inspection at FSSA's Office of General Counsel, Indiana Government Center South, 402 West Washington Street, Room W451, Indianapolis, Indiana.
