Notice of Public Comment Period for
Hoosier Care Connect 1915(b) Waiver Renewal


In accordance with public notice requirements established at 42 CFR 447.205, the Indiana Family and Social Services Administration (FSSA), Office of Medicaid Policy and Planning (OMPP) publishes this notice of the renewal application of Indiana's Hoosier Care Connect 1915(b) Managed Care Waiver. The public notice period will run 6/29/2022 to 7/29/2022.

Hoosier Care Connect is a coordinated care program for Hoosiers aged 65 and over, or with blindness or a disability, as well as some groups of children in foster care, who live in the community and are not eligible for Medicare. Members of the program receive all Medicaid-covered benefits in addition to care coordination services. Care coordination is a client-centered, assessment-based approach to integrating health care and social support services and is offered to all Hoosier Care Connect members through disease management, care management, and complex care management.

The Hoosier Care Connect 1915(b) Waiver renewal only lengthens the duration of the program. The renewal does not contain any modifications to the program.

A copy of this notice is now on file at the Indiana Government Center South, 402 West Washington Street, Room W451, Indianapolis, Indiana and is open for public inspection. Also, a copy of this notice is now available and may be inspected with the local county Division of Family Resources office, except in Marion County, where public inspection may be made at 402 West Washington Street, Room W374, Indianapolis, Indiana.

There will be no public hearing. Anyone who wishes to comment on the proposed renewal may submit written comments through July 29, 2022, at 5:00 p.m. Written comments may be directed to FSSA, Office of Medicaid Policy and Planning, Attention: Madison May-Gruthusen, 402 West Washington Street, Room W382, P.O. Box 7083, Indianapolis, IN 46207-7083, or by email to spacomments@fssa.in.gov. Correspondence should be identified in the following manner: COMMENT RE: HOOSIER CARE CONNECT WAIVER RENEWAL. Written comments will be made available for public inspection at the address herein of the OMPP.
