In accordance with public notice requirements established at 42 CFR 447.205, the Indiana Family and Social Services Administration, Office of Medicaid Policy and Planning (OMPP) publishes this notice of the renewal application of Indiana's Hoosier Care Connect 1915(b) Managed Care Waiver.

PURPOSE

Hoosier Care Connect is a coordinated care program for Hoosiers age 65 and over, or with blindness or a disability, as well as some groups of children in foster care, who live in the community and are not eligible for Medicare. Members of the program receive all Medicaid-covered benefits in addition to care coordination services. Care coordination is a client-centered, assessment-based approach to integrating health care and social support services and is offered to all Hoosier Care Connect members through disease management, care management, and complex case management.

The Hoosier Care Connect 1915(b) waiver renewal only lengthens the duration of the program. The renewal does not contain any modifications to the program.

SUBMISSION OF COMMENTS

A copy of this notice is now on file at the Indiana Government Center South, 402 West Washington Street, Room W451 and is open for public inspection. Also, a copy of this notice is now available and may be inspected by contacting the local county Division of Family Resources office, except in Marion County, where public inspection may be made at 402 West Washington Street, Room W374, Indianapolis, Indiana.
