Under IC 4-22-2-24, notice is hereby given that on September 3, 2009, at 10:00 a.m., at the Indiana State Department of Health, 2 North Meridian Street, Indianapolis, Indiana, the Indiana State Department of Health will hold a public hearing on proposed amendments to 410 IAC 1-1 to modify the list of communicable diseases that require documentation of immunity, modify documentation requirements and requirements concerning statements of immunization histories, and add updates concerning school reporting forms.

The Indiana State Department of Health has the authority to adopt this rule under IC 16-19-3-5 and IC 20-34-4-2. This rule will not impose any additional requirements or costs under IC 4-22-2-24(d)(3).
