Ownership / Administrator Changes


A change in the ownership or the administration of a home health agency may affect the quality and quanitity of services that are available. The change may be reflected in a positive or a negative manner. Changes in the ownership or administration of a home health agency must be made known to the Indiana State Department of Health. This information is entered into our files, and a history is maintained.

Up to three sets of ownership and administrator change data may be presented on each report. The data is presented as a table, where each data column may be identified by a date that corresponds to one of the State Licensure Survey dates. If a date is not presented at the head of the column, the positions within the column are not applicable ("N/A"). If a date appears, the period covered is from that date to the date in the column to the right, or to the date of the first data that was collected on the agency. If an administrator change or an ownership change occurred within that period, a "Yes" will appear within the column. If no change occurred, a "No" will appear in the column.

This table indicates that there has been at least one ownership change and one administrator change since the agency became known to the Department of Health. The ownership change occurred between July 9, 1996, and July 14, 1999. It occurred between the date of the column that shows the change and the date associated with the column to the right. There was also an administrator change, and that administrator change occurred between the date the agency became known to the Department (that date is not noted), and July 9, 1996, the date of the 2nd most recent survey.
