Home Health Agency Consumer Reports Search Page

To view a consumer report for facilities within a specific city, choose the first letter of the city name on the alphabet bar under the heading City Search. This will take you to a list of all the cities that begin with that letter, and you can select your city of interest there. Once you click on a city, you will be taken to a list of all the facilities within that city. At this point, simply select the facility of interest and you will be taken to the consumer report for that facility.

To begin a search for a facility which services a specific county, please click on the county on the map. This will take you to a list of all the facilities that service that county. As before, by clicking on a facility, you will be taken to the consumer report for that facility. You can also choose the first letter of the county name on the alphabet bar under the heading County Search. This will take you to a list of all the counties that begin with that letter, and you can select a county of interest there.
