Vital Records
The Indiana State Department of Health Division of Vital Records is responsible for maintaining and issuing certified copies of vital records, including birth, death, and fetal death certificates for events that occurred in Indiana. The office of Vital Records officially began recording births in 1907, and death recordings began in 1900.

Ordering Certified Records
To apply for a birth certificate by mail and pay by check or money order:
- Complete State Form 49607
- Cost for the first certificate is $10.00, and $4.00 for each additional copy
- Make checks payable to Indiana State Department of Health
- Average Processing Time: 20 business days
To apply for a death certificate by mail and pay by check or money order:
- Complete State Form 49606
- Cost for the first certificate is $8.00, and $4.00 for each additional copy
- Make checks payable to Indiana State Department of Health
