The Indiana State Department of Health (ISDH) is responsible for the licensing of health care facilities. The ISDH’s Health Care Quality and Regulatory Commission serves as the state survey agency for state licensing and federal Medicare/Medicaid certification. This web page is intended to provide access to reports prepared by the Commission pertaining to health care facilities.

ISDH Health Care Facility Consumer Reports
In 2003 the ISDH created its first health care facility consumer report. That report was for nursing homes. The ISDH has since added additional consumer reports for other provider types. Each consumer report provides information about health care facilities and entities. The information includes location, contact information, administrators, services provided, and survey history.
Comprehensive Care Facilities (Nursing Homes)
The Comprehensive Care Facility Bed Count Report provides the number of licensed and certified beds in nursing homes. The report includes breakdowns by county. The report is produced quarterly.

The Comprehensive Care Facility Occupancy Rate Report provides the occupancy rate for nursing homes. The report includes breakdowns by county. The reports are produced for January 1 and July 1 of each year and based on facility-reported data specific for those dates.
Residential Care Facilities (Licensed Assisted Living)
The Residential Care Facility and Bed Count Report provides the number of licensed residential care facilities, number of residential care beds, and occupancy rates. The report includes breakdowns by county. The reports is produced for January 1 and July 1 of each year and based on facility-reported data specific for those dates.
The Residential Care Facility Occupancy Rate Report provides the occupancy rate for residential care facilities. The report includes breakdowns by county. The reports are produced for January 1 and July 1 of each year and based on facility-reported data specific for those dates.
The Indiana Medical Error Report provides the number of reported serious adverse events at hospitals, ambulatory surgery centers, abortion clinics, and birthing centers. The report is based on the National Quality Forum’s Serious Adverse Events. The report is produced annually.
