Reports on Health Care Facilities

The Indiana Department of Health (IDOH) is responsible for the licensing of health care facilities. The Consumer Services and Health Care Regulation Commission within IDOH serves as the state survey agency for state licensing and federal Medicare/Medicaid certification. This web page is intended to provide access to reports prepared by the Commission pertaining to health care facilities.

Announcement

House Enrolled Act (HEA) 1004 requires each hospital’s 2022 fiscal report to be updated with the additional required data. As stated in HEA 1004, “If a hospital submitted the hospital's report for 2022 before July 1, 2023, the hospital must submit a revised report with the data set forth in subsection (a)(7) through (a)(9) before December 1, 2023.”

Ambulatory Surgery Centers (ASC)

Medical Error Reports

  • Indiana Medical Error Report - The Indiana Medical Error Report provides the number of reported serious adverse events at hospitals, ambulatory surgery centers, abortion clinics, and birthing centers. The report is based on the National Quality Forum’s Serious Adverse Events. The report is produced annually.

Health Facilities Consumer Reports

  • IDOH Health Care Facility Consumer Reports - In 2003 IDOH created its first health care facility consumer report. That report was for nursing homes. IDOH has since added additional consumer reports for other provider types. Each consumer report provides information about health care facilities and entities. The information includes location, contact information, administrators, services provided, and survey history.
