Reports on Health Care Facilities

The Indiana Department of Health (IDOH) is responsible for the licensing of healthcare facilities. The Consumer Services and Healthcare Regulation Commission within IDOH serves as the state survey agency for state licensing and federal Medicare/Medicaid certification. This page is intended to provide access to reports prepared by IDOH pertaining to healthcare facilities.

The Consumer Reports are currently undergoing updates and may not accurately reflect certain facility data at this time. IDOH is actively working to resolve these issues with a targeted completion date of October 17, 2025.

IDOH maintains consumer reports on licensed healthcare facilities and entities. Each consumer report provides information about these entities, including location, contact information, administrators, services provided, and survey history.

Consumer reports are available for:

  • Hospitals
  • Hospice Agencies
  • Home Health Agencies
  • Ambulatory Surgery Centers (ASCs)
  • End-Stage Renal Disease Clinics (Dialysis Clinics)
  • Intermediate Care Facilities for Individuals with Intellectual Disabilities
  • Nursing Homes (Comprehensive Care Facilities)
  • Residential Care Facilities (Licensed Assisted Living Facilities)
