Step 1

Select and click on the applicable application for the project:

Permit Application for Construction or Alteration of an Acute Care Facility

  • Hospital
  • Ambulatory Outpatient Surgical Center
  • Primary Care Outpatient Facility
  • Excluded Rehabilitation Unit
  • Excluded Psychiatric Unit

Permit Application for Construction or Alteration of a Long Term Care Facility

  • Comprehensive Care
  • Residential Care
  • Community Residential Facility for the Developmentally Disadvantaged/Intellectually Disables

Permit Application for Construction or Alteration of a School Facility

  • Pre-K/Elementary School
  • Secondary /High School
  • Charter Schools
Note: Post-secondary schools do not require plan review.

Permit Application for Construction or Alteration of a Child Care Facility

  • Children’s Group Home/Private Secure Homes
  • Child Care Institutions

Permit Application for Construction or Alteration of a Mental Health Institution

  • Private Mental Health Institutions licensed under the Division of Mental Health (DMH), Family Social Services Administration (FSSA)

Step 2

Print the selected application.

Step 3

Complete the application.

Step 4

Send the completed application [with the appropriate fee (if any) by check or money order] as shown:

DO NOT SUBMIT DRAWINGS OR PLAN SETS WITH THE APPLICATION AND FEES.

Step 5

Upon receipt and processing, the Indiana State Department of Health will contact the architectural/engineering firm with a confirmation email. The email will include the assigned project number and instructions on the submission of all required documentation.

Step 6

Architectural/Engineering firm will submit the required documentation in step 5 electronically following the instructions included in the email from step 5.

Step 7

Indiana State Department of Health will assign the project for review when all documents are present. If documents are missing or unusable, the architect/engineer will receive a request for the missing information. No project will be assigned for review until all documents are present.
