Care Coordination
Children who have special health care needs often require more health care services than typically healthy children. Getting the necessary services to maintain your child's health can involve dealing with many different health care providers, state agencies and local community resources. Managing the health care system can sometimes be confusing and overwhelming for parents who are trying to cope with their child's complex medical issues.
That is why Indiana’s Children’s Special Health Care Services (CSHCS) offers care coordination services to help guide Indiana’s families through these systems of care. CSHCS care coordinators are nurses and social workers who work with the child's family and doctors to identify needs and the resources available to meet these health care needs.
Examples of our services include:
- Assistance in identifying financial resources
- Assist families in identifying specialty care and community resources
- Educating families on the various medical waiver programs
- Assistance with locating peer support groups and community support services in their area
- Educating families on adolescent transition to adult health care and services
- Assistance with special education issues

- Families of children who may be facing a new diagnosis
- Families with a child/youth facing a major transition in service
- Families with limited financial resources
- Families who have recently moved to Indiana
- Families with a newly adopted child with special health care needs
